Parent and/or Legal Guardian Written Consent to participate in the Ningin Wonder Girls Dance Contest Instructions:

Parents and/or legal guardians please read this Consent Form and the Contest Rules at http://jyp.ningin.com/contest carefully. If you wish to give consent, please completely and sign this Consent Form, and return the signed and completed form by February 26th. Submissions that are not accompanied by properly signed and completely Coznsent Forms and any other necessary information or materials will not be accepted by Ningin. Note: Contestants who are 18 or over do not need to submit this consent form.

1. Enter and participate in the Ningin Wonder Girls Dance Contest. I (we) acknowledge that I (we) have read and understand the Contest Rules, and that I (we) and the Entrant will be bound by these Contest Rules.

2. Allow the Entrant’s name to be displayed along with their video entries.

3. Participate in any media or promotional events, including any possible winner ceremonies, media interviews, and publicity events, related to the Contest.

4. I (we) confirm that the contestant is a citizen or legal resident of the United States or Canada (excluding Quebec) will report any prize winnings to all relevant federal, state and local tax authorities.

5. Privacy Disclaimer: Any personal information collected during the course of the Contest by Ningin will only be used for administering this competition, or as otherwise set out in the Contest Rules. Except where prohibited by law, participation in the Contest constitutes the Entrant’s and his or her parent’s/legal guardian’s consent to the storage, use and disclosure of the Entrant’s entry details as set out in the Contest Rules.
